Background
The STOP AIDS Project has been conducting paper-based
Behavioral Risk Assessment surveys for 21 years. Data is utilized
by the San Francisco Department of Public Health, the California
Office of AIDS, and Johns Hopkins. Case studies of

Surveying
The San Francisco Department of Public Health
collected the paper survey forms, entered them into a stand-alone
database, and provide summary reports several months later. In
February, 2006, STOP AIDS developed it’s own Filemaker
database to collect the data via data entry screens, and
web form input, reducing the reporting turning time to 2
weeks.

Palm Survey
In January, 2007, STOP AIDS rolled out a Palm-based survey tool
called ReForm made by AnyWare
Mobile Solutions. ReForm
is a web-based survey design tool. A small Palm software
client is downloaded via the web or syncing with a desktop computer
to WiFi-enabled Palms. The Reform Palm client is then used
to download the survey forms to the Palms.
Surveys are conducted
on the Palms and either immediately uploaded back to the ReForm
website, or uploaded later when back in range of a WiFi connection. Reports
can then be generated via the web or data can be exported to Excel,
CSV, or XML. Currently, we export data in CSV format and import
into our master Filemaker survey database.

Currently, branching logic must
be implemented by AnyWare developers for an additional fee. Future
